new patient forms


  • Paperwork must be done at least 48 hours before your first visit

payment / fees


  • We accept payment at time of service. No exceptions. We will submit to health insurance as a courtesy once. Follow up is up to you.
  • If order to confirm your first appointment, we take a $100 credit card deposit, which will be applied to your first visit. If you book online, we will call you for a credit card number.
  • If you give us at least 24 hours notice to reschedule, great, we will still apply to the first visit. If you cancel, and do not reschedule, only $85 will be refunded. The $15 is to cover credit card processing fees. If you cancel and do not give us 24 hours notice, there will be no refund.
  • If we have tried and not been able to get your credit card number, your appointment may be given away, as we normally have a waiting list.
  • Our fist visit cost is $200. It takes 60-75 minutes. During that time, we do a history, exam, treatment/adjustment, and any therapies we think are indicated.
  • Exercises, if applicable, will be sent later, after your notes and report are done.
  • Thereafter, visits are $85 for a full body adjustment, and one therapy to one area. Additional therapies are $25 each.
  • We will not just add therapies without checking, however discussion of fees during an appointment interrupts the flow of care. So, please know this going in. If you request additional therapies, fees will be added.
  • When you finish your appointment, we will send you to our front desk, where there is an iPad so you can schedule your next appointment. Or, you can go to our website, miluschiropractic.com→ to schedule.
  • We recommend people come back in a timely fashion in accordance with many years of treatment protocols and outcomes. If you choose not to comply, our results are not as effective. That is up to you, but, we cannot be held responsible for your outcome.

cancellations / rescheduling


  • We need 24 hours notice to cancel, no exceptions.
  • To cancel or reschedule, text to 925-858-4375 or email drjen@miluschiropractic.com. Note: Our booking system does not handle cancellations or rescheduling.
  • Inside of 24 hours, you will be charged as if you had come to appointment.




insurance


  • We are out of network for every insurance company. The reason for this? Insurance companies never seem to pay, and we don’t have time to chase down the $29-$60 per visit that they pay if we beg enough.
  • Doctors who accept insurance and are in network spend only 5-8 minutes with a patient. We spend 20-60, depending on the visit.
  • We are happy to take your insurance information and submit as a courtesy. Once. We will not chase claims, that is up to you. We will send you all records and receipts necessary to pester them until you get your money back.




personal injury cases


  • We want to help as many people as possible get better when they are injured. However, patients need to take responsibility for their care. We are happy to take these cases, but, we cannot take a lien. We suggest, if you need funding for your recovery, to apply for CareCredit, a healthcare financing credit card.




emergency appointments


Emergency appointments are available at the ER or Urgent Care. We are neither. We really want to help, but, good things take time. If you are in such terrible pain that you need an ER, please go. We treat acute low back pain and neck pain, but, only as our schedule allows.
