  • new patient forms

  • payment / fees
    We accept payment at time of service. FSA & HSA cards are accepted. Unfortunately, we are not in network with any insurance companies. We are able to provide patients with an itemized super bill. Patients will then be able to submit their own claim for a partial/full refund from insurance for their visits. Our first visit cost is $200. It takes 60-75 minutes. During that time, we do a history, exam, treatment/adjustment, and any therapies we think are indicated. Exercises, if applicable, will be sent later, after your notes and report are done. Thereafter, visits are $85 for a full body adjustment, and one therapy to one area. Additional therapies are $25 each. We recommend people follow our discussed treatment plan, at home care, and other recommendations we make in order to ensure speedy recovery.

  • cancellations / rescheduling
    As our cancellation policy states, we require a 24 hour notice for cancellations. If you need to cancel within that 24 hour window or you do not show up to your appointment time without notice, you will be charged the $85 visit fee.   • emergency appointments
    If you are having an emergency, please call 911 or visit your nearest emergency room or urgent care. If you decide coming to see a chiropractor is more suitable for your pain complaint, contact us directly using the phone number provided at the end of the page, we will try our best to fit you into our schedule as soon as possible.
